How do you define what is, or is not, a medication error?
Medication errors can most simply be described as involving too much, too little or the wrong kind of medication. For instance, any time that a resident of a nursing home or similar type of health care facility is prescribed a medication, his or her care provider is responsible to make certain that the individual actually takes his or her prescription.
What are the underlying causes?
Certainly, staffing issues can cause some medication errors to occur. In some instances, overworked doctors might prescribe the wrong dosage of a particular medicine, or fail to notice that the drug they are prescribing is contra-indicated and should not be used in conjunction with medications the resident is already taking. In others, simple neglect and lack of care in administering medications, in reading patient charts or in performing other duties may be to blame.
